Travel Insurance Claim Procedure

  • Gather supporting documents. Collect all required documents specific to your type of claim.
  • Complete and submit claim form. Download the claim form and complete it. Submit the completed form along with all supporting documents.

Remark: Please provide the original copy when the Company requests to consider a claim.

  • Copy of Passport
  • E-ticket/boarding pass of departure and arrival flights from Thailand
  • Full Medical report mentioned Chief Complaint, Diagnosis and Treatment Details
  • Receipt of medical expenses and Breakdown Invoice/Detailed Medical expenses
  • Copy of prescription/Picture of Prescripted Medications
  • In case of In-patient, please provide further document as below
    – Admission Note, Doctor Order and Progress Note
  • Copy of the first page of the Insured’s passbook (for indemnity payment transfer)

  • Copies of medical report or death certificate from a qualified physician treating the Insured Person, Insured Person’s Immediate Family or Close Business Associate, copy of court witness summon
  • Proof of relation of Insured Person and Insured Person’s Immediate Family or Close Business Associate
  • Documentary proof showing any deposit, travel fare and accommodation charges as well as copies of additional travel and hotel accommodation charges incurred
  • Proof of non-refundable pre-book travel fare and accommodation charges and other charges incurred (if any)
  • Copy of the first page of the Insured’s passbook (for indemnity payment transfer) or the executor’s passbook with the proof of executor status
